Primary Care Physician (MD/DO) – Summary
- Provide comprehensive primary care to adults 65+, focusing on preventive medicine and chronic disease management.
- Work in a value-based care model that emphasizes quality, outcomes, and patient satisfaction.
- Collaborate with an integrated care team to deliver holistic and coordinated patient care.
- Enjoy a balanced patient census, allowing time for thorough visits and meaningful interactions.
Duties & Responsibilities
- Deliver primary care services to a panel of Medicare Advantage patients, focusing on preventive and chronic care management.
- Conduct patient visits averaging 12–15 per day, with 20–40 minutes allocated for each appointment to ensure thorough care.
- Lead and collaborate with a multidisciplinary team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, and Pharmacy personnel.
- Utilize electronic medical records (EMR) and documentation best practices for efficient and accurate patient charting.
- Engage in ongoing professional development, including comprehensive onboarding and value-based care training.
- Coordinate with referral and support services to provide seamless patient experiences.
- Promote a mission-driven culture focused on improving health outcomes for older adults.
Salary & Benefits
- Competitive base salary ranging from $235,000 to $270,000, commensurate with experience.
- Annual performance bonus up to 20% of base salary.
- Commencement and early sign-on bonuses available.

- Total first-year compensation exceeding $300,000, with additional earning potential based on performance.
Qualifications & Requirements
- MD or DO degree required.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience in geriatric medicine preferred; HEDIS and value-based care experience a plus, though not mandatory.
- Commitment to delivering patient-centered, high-quality care.
- Strong communication and interpersonal skills.
