Primary Care Physician (Internal Medicine or Family Medicine, MD/DO) Job OverviewWe are seeking a dedicated Primary Care Physician (MD/DO) with training in Internal Medicine (IM) or Family Medicine (FM) to join our established private practice in Aventura, FL. This full-time role provides an opportunity to practice comprehensive primary care in a supportive, patient-focused environment, serving both Private Medicare and private patients. As a valued member of our healthcare team, you will be instrumental in delivering high-quality, value-based care and managing chronic conditions for a diverse patient panel.
Responsibilities- Provide comprehensive primary care to adult patients with a focus on prevention, diagnosis, and management of acute and chronic illnesses.
- Conduct thorough patient assessments, develop individualized care plans, and ensure continuity of care through regular follow-up visits every 3-4 months.
- Spend approximately 30 minutes with each patient, ensuring a thorough and compassionate approach to care.
- Manage an average daily patient load of 12-15, supported by a team that includes one Medical Assistant (MA) and front-end personnel.
- Utilize three exam rooms to facilitate efficient patient flow and optimal care delivery.
- Participate in a shared call rotation with other physicians for after-hours patient needs.
- Take hospital admissions as required, ensuring seamless transitions of care for admitted patients.
- Apply value-based care principles in daily practice, working within a model that combines value-based care and Relative Value Units (RVUs).
- Independently manage chronic conditions without frequent referrals, using best practices and evidence-based guidelines.
- Document patient encounters accurately and completely in accordance with practice and regulatory requirements.
- Medical degree (MD or DO) with completion of an accredited residency in Internal Medicine or Family Medicine.
- Board certification in Internal Medicine (IM) or Family Medicine (FM) required.
- Minimum of two years post-residency clinical experience, with proven ability to practice independently.
- Experience managing chronic conditions in an outpatient setting.
- Ability to admit patients to the hospital and manage their care during hospitalization.
- Value-Based Care (VBC) experience or an understanding of VBC metrics is preferred.
- Russian language skills are preferred, but not required.
- Practice hours: Monday through Friday, 9:00 AM – 5:00 PM. Closed on Saturdays and Sundays.
- After-hours responsibilities are covered through a shared call system among physicians.
- Work in a collaborative environment with access to three exam rooms and dedicated support staff.
