Primary Care Physician (Internal Medicine or Family Medicine, MD/DO) - Summary
- Full-time position serving a senior patient population in Cleveland, OH.
- Focused on value-based, patient-centered primary care delivery.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, primarily managing 3-5 chronic conditions per patient.
- Conduct thorough patient assessments, develop individualized care plans, and deliver ongoing management of acute and chronic illnesses.
- Work collaboratively with a dedicated care team including medical assistants, schedulers, and referral specialists to ensure seamless patient care and communication.
- Participate in value-based care initiatives, focusing on improving patient outcomes and quality metrics.
- Coordinate care with in-house consulting specialists, including cardiology and podiatry, as well as holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain clear and accurate medical records and documentation in compliance with organizational and regulatory standards.
- Allocate administrative time within your clinic schedule to support effective patient management and follow-up.
- Participate in team meetings, quality improvement activities, and ongoing professional development.
Salary & Benefits
- Competitive base salary ($250,000+), with additional performance-based incentive bonuses and a signing bonus opportunity.
- Partnership track available within 12-18 months, with no buy-in required; approximately 65% of physicians achieve partnership status after one year.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, compassionate care to a senior patient population.
- Experience with value-based care models and chronic disease management preferred.
- Excellent interpersonal, communication, and teamwork skills.
- Eligibility to obtain and maintain an active medical license in Ohio.
