Primary Care Physician (PCP) - Summary
- Full-time physician opportunity in a mission-driven, physician-led organization.
- Focus on treating senior patients with chronic conditions in an outpatient setting.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, focusing on those with 3-5 chronic diseases.
- Conduct patient visits (approximately 18 per day), including preventative care, chronic disease management, and follow-up care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist.
- Work closely with in-house consulting specialists such as cardiology and podiatry to coordinate patient care.
- Participate in clinic hours from Monday to Friday, 7:30 am to 5:00 pm, with the last patient scheduled at 4:15 pm.
- Engage in value-based care initiatives to ensure quality and efficiency in healthcare delivery.
- Utilize administrative time built into the schedule for documentation and patient management.
- No night or weekend shifts required.
- Maintain compliance with all organizational policies and healthcare regulations.

Qualifications & Requirements
- Completed residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
