In this Primary Care Physician position, you will be part of an integrated care team that includes nurse practitioners, physician assistants, medical assistants, social workers, and pharmacy and lab professionals. This collaborative model fosters comprehensive care coordination and ensures that each patient benefits from a tailored and holistic approach to their health. The organization’s commitment to value-based care allows physicians to spend more time with each patient, with an average of 12–15 patients per day and extended visit lengths of 20–40 minutes. This enables you to focus on preventive care, chronic disease management, and truly personalized medicine.

Primary Care Physician – Internal Medicine/Family Medicine (VBC, Geriatrics) – Summary- Full-time opportunity focused on value-based, preventive care for adults 65+ (primarily Medicare Advantage patients)
- Be part of an integrated care team (NP/PA, MAs, social workers, pharmacy, and lab support)
- Average 12–15 patients per day with 20–40 minute visits for thorough, personalized care
- Comprehensive onboarding and professional development, including value-based care training
- Provide primary care services for adults aged 65+, focusing on preventive care and chronic disease management
- Develop individualized care plans and coordinate with the integrated care team for optimal patient outcomes
- Document patient encounters following best practices and EMR optimization standards
- Participate in value-based care initiatives and quality improvement programs
- Engage in ongoing training and professional development activities
- Contribute to a collaborative, impact-driven work culture dedicated to improving senior health

- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Geriatric experience preferred but not required
- HEDIS experience is a plus
- Value-Based Care experience is preferred; comprehensive training provided for those new to this model
