Primary Care Physician (IM/FM/PCP) – Summary
- Full-time outpatient primary care position focused on senior populations

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 senior patients, most with multiple chronic diseases
- Conduct patient visits (approximately 18 per day) within standard clinic hours
- Collaborate with a dedicated support team including Medical Assistants, Schedulers, and Referral Specialists
- Utilize in-house consulting specialists in fields such as Cardiology and Podiatry
- Integrate holistic health services such as Acupuncture, Tai Chi, Yoga, and Nutrition into patient care plans
- Maintain accurate and timely documentation of patient interactions
- Participate in value-based care initiatives to improve patient outcomes
- Engage in care coordination and administrative time allotted within the work schedule
- Adhere to all organizational protocols and compliance standards
- No night or weekend clinical hours required

Qualifications & Requirements
- MD or DO degree from an accredited institution
- Completion of an accredited residency in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to providing compassionate, high-quality care to senior patients
- Strong interpersonal, communication, and organizational skills
- Experience or interest in value-based care models preferred
