Primary Care Physician (IM/FM/PCP) – Summary- Full-time outpatient primary care position focused on senior populations
- Patient-centered model emphasizing value-based care
- Small patient panels for personalized attention
- Collaborative, physician-led organization
- Provide comprehensive primary care to a panel of up to 450 senior patients, most with multiple chronic diseases
- Conduct patient visits (approximately 18 per day) within standard clinic hours
- Collaborate with a dedicated support team including Medical Assistants, Schedulers, and Referral Specialists
- Utilize in-house consulting specialists in fields such as Cardiology and Podiatry
- Integrate holistic health services such as Acupuncture, Tai Chi, Yoga, and Nutrition into patient care plans
- Maintain accurate and timely documentation of patient interactions
- Participate in value-based care initiatives to improve patient outcomes
- Engage in care coordination and administrative time allotted within the work schedule
- Adhere to all organizational protocols and compliance standards
- No night or weekend clinical hours required
