Primary Care Physician - Summary
The Primary Care Physician will deliver comprehensive outpatient care to a panel of Medicare Advantage patients, focusing on preventive health, chronic disease management, and wellness. Physicians in this role benefit from extended patient interaction times, a manageable daily census, and a robust integrated care team. Duties & Responsibilities
- Provide high-quality, evidence-based primary care to adults aged 65 and older within an outpatient setting.
- Manage a manageable daily patient census (typically 12–15 patients per day), allowing for comprehensive care and meaningful relationships with patients.
- Conduct thorough patient assessments, develop personalized care plans, and deliver preventive, chronic, and acute care services.
- Collaborate with an interdisciplinary care team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, pharmacy staff, and referral coordinators, to ensure holistic patient care.
- Utilize extended visit times (20–40 minutes per visit) to address complex medical and psychosocial needs of senior patients.
- Participate in a supportive onboarding process, including a comprehensive three-week "Physician Learning Journey" focused on value-based care, EMR optimization, and documentation best practices.
- Engage in continuous quality improvement initiatives and contribute to the achievement of value-based performance metrics.
- Document all patient interactions and clinical activities accurately and efficiently in the electronic medical record (Athena, in transition).
- Foster a culture of compassion, respect, and collaboration within the care team and with patients and their families.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine.
- Experience with geriatric care and/or Medicare Advantage populations is highly valued.
- Previous exposure to Value-Based Care models and HEDIS quality measures is preferred but not required; on-the-job training is provided for motivated candidates.
- Demonstrated commitment to patient-centered, preventive healthcare for seniors.
- Active, unrestricted medical license to practice medicine.
