Primary Care Physician - Summary
- Deliver comprehensive primary care for adults aged 65+, with a focus on preventive medicine and chronic disease management.
- Work as part of an integrated care team, including Nurse Practitioners/Physician Assistants, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordinators.
- Practice in a value-based care model designed to improve patient outcomes and provider satisfaction.
Duties & Responsibilities
- Manage a patient panel of adults 65+ (primarily Medicare Advantage), focusing on both preventive and chronic care management.
- See an average of 12-15 patients per day, allowing for extended visit times (20-40 minutes per appointment) to ensure thorough, patient-centered care.
- Collaborate closely with a multidisciplinary care team to coordinate diagnostics, referrals, and follow-up care.
- Utilize electronic health records and data-driven tools to monitor patient outcomes and support quality improvement initiatives.
- Participate in organization-wide efforts to meet and exceed quality metrics, such as HEDIS measures, to support optimal health outcomes.
- Engage in ongoing professional development and team-based learning, with training provided in value-based care principles if needed.
- Play an active role in shaping a positive, impact-driven clinical environment that prioritizes both patient and provider well-being.

- Highly competitive base salary (commensurate with experience), with total first-year compensation exceeding $300,000 for qualifying candidates.

Qualifications & Requirements
- MD or DO degree from an accredited medical school.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience in geriatric medicine, HEDIS measures, or value-based care is preferred but not required (training provided).
- Current and unrestricted medical license to practice as a Primary Care Physician in the relevant state.
