Primary Care Physician - Summary
- Deliver comprehensive, preventive primary care to adults 65+, focusing on chronic disease management and wellness in a value-based care environment.

Duties & Responsibilities
- Provide primary medical care to a panel of adult patients, emphasizing preventive health and chronic disease management.
- Conduct patient visits averaging 20 to 40 minutes, allowing for thorough evaluations and meaningful patient interactions.
- Manage a daily patient census of 12 to 15 individuals, supporting optimal patient outcomes.
- Work collaboratively with nurse practitioners, physician assistants, medical assistants, social workers, lab and pharmacy staff, and referral coordinators as part of an integrated care team.
- Participate in comprehensive onboarding, including training in value-based care, EMR optimization, and best practices for documentation.
- Maintain accurate and timely medical records in accordance with organizational and regulatory standards.
- Engage in ongoing professional development to stay current with advancements in primary care and value-based healthcare delivery.
- Demonstrate commitment to improving health outcomes for older adults while upholding organizational values and clinical excellence.

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Geriatric experience is preferred but not required.
- Prior experience with HEDIS measures is a plus.
- Experience in value-based care is preferred; comprehensive training is available for those new to this care model.
