Primary Care Physician - Summary
- Full-time position focusing on preventive and chronic care for adults aged 65+

Duties & Responsibilities
- Provide comprehensive primary care for a senior adult patient population, mostly Medicare Advantage
- Diagnose, treat, and manage chronic diseases with a preventive approach
- Conduct patient visits lasting 20–40 minutes to ensure thorough evaluation and personalized care
- Maintain a daily patient census of 12–15
- Collaborate with nurse practitioners/physician assistants, medical assistants, social workers, and pharmacy staff
- Participate in integrated care team meetings and referrals to optimize patient outcomes
- Document all patient interactions in the electronic medical record (EMR)
- Engage in ongoing learning and professional development, including value-based care training and EMR optimization
- Support and contribute to a mission-driven, impact-focused clinical environment

Qualifications & Requirements
- MD or DO degree
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Geriatric experience preferred but not required
- Experience with HEDIS quality measures a plus
- Interest or background in value-based care models preferred; training provided for those without previous experience
- Active and unrestricted medical license in the relevant state (or eligibility to obtain)
