Primary Care Physician (PCP) - Full-Time
Join a leading value-based care organization as a dedicated Primary Care Physician, focusing on delivering exceptional preventive care for adults aged 65 and older. As a Primary Care Physician, you will work collaboratively within an integrated healthcare team to ensure each patient receives the highest quality of care. As a Primary Care Physician, you will work with a diverse team of healthcare professionals including Nurse Practitioners (NPs), Physician Assistants (PAs), Medical Assistants, Social Workers, and in-house laboratory and pharmacy support. Your daily patient census is kept intentionally low (12-15 patients), allowing for longer appointment times (20-40 minutes each) and a more holistic approach to each individual's health needs. The majority of your patient population will be Medicare Advantage recipients, with an emphasis on chronic disease management and preventive care. This patient-focused model supports your professional growth through comprehensive onboarding and ongoing training, including value-based care best practices, electronic medical record (EMR) optimization, and integrated care workflows.

- Geriatric experience preferred
- HEDIS experience is a plus
