Primary Care Physician – Summary
- Deliver comprehensive primary care to adults aged 65+ with a focus on preventive medicine and chronic disease management.

Duties & Responsibilities
- Provide high-quality, preventive, and chronic care management to a patient panel of adults 65+, primarily Medicare Advantage members.
- Conduct 12–15 patient visits per day with extended appointment times (20–40 minutes each), allowing for thorough evaluations and meaningful interactions.
- Collaborate closely with multidisciplinary care teams, including Nurse Practitioners/Physician Assistants, Medical Assistants, Social Workers, laboratory, and pharmacy staff to coordinate comprehensive patient care.
- Participate in value-based care initiatives, focusing on improving HEDIS measures and overall patient health outcomes.
- Document all patient care activities accurately and efficiently utilizing an evolving EMR system (Athena under transition).
- Engage in a 3-week Physician Learning Journey that includes specialized training in value-based care, documentation best practices, and workflow optimization within an integrated care model.
- Foster a supportive, impact-driven clinical environment where patient satisfaction and quality of care are paramount.
Salary & Benefits
- Competitive base salary ranging from $235,000 to $270,000, based on experience and clinic assignment.
- Potential for total first-year compensation exceeding $300,000, including performance and commencement bonuses.
- Annual performance bonus up to 20% of base salary based on quality outcomes and patient satisfaction.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine required.
- Experience in geriatric care or with senior patients preferred.
- Familiarity with HEDIS measures is a plus.
- Prior experience in value-based care is advantageous, though not required; comprehensive training provided.
- Eligibility to obtain and maintain an active medical license in the state of practice.
