As a Primary Care Physician, you will be responsible for managing a patient panel composed primarily of Medicare Advantage members, with an emphasis on chronic disease management and preventive care. Primary Care Physician - Summary
- Full-time opportunity for a Board Certified or Board Eligible Internal Medicine or Family Medicine physician
- Focus on preventive care and chronic disease management for adults 65+

Duties & Responsibilities
- Provide comprehensive primary care to a patient panel of adults aged 65 and older
- Conduct visits focused on preventive care, chronic disease management, and overall wellness
- Maintain an average daily census of 12-15 patients, allowing for 20-40 minute appointments per visit
- Collaborate with an integrated care team, including advanced practice providers, medical assistants, social workers, lab and pharmacy staff
- Utilize value-based care principles and documentation best practices
- Participate in ongoing professional development and training, including onboarding in value-based care and EMR optimization
- Engage with patients and families to build strong, long-term relationships and support positive health outcomes

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Experience in geriatric medicine preferred
- HEDIS experience is a plus
- Interest in or experience with value-based care models (training provided as needed)
