Primary Care Physician (MD/DO) – Full-time

As a Primary Care Physician, you will practice in a patient-centered clinical model that prioritizes quality outcomes over volume. You’ll have the support of a multidisciplinary care team, access to state-of-the-art resources, and a schedule that allows for meaningful interactions with each patient. This role is ideal for board-certified or board-eligible Internal Medicine or Family Medicine physicians, including those with experience or interest in Value-Based Care (VBC).

Primary Care Physician – Summary

Duties & Responsibilities
- Deliver comprehensive primary care services to a patient panel of adults aged 65 and older
- Manage chronic conditions and preventive health for Medicare Advantage patients
- Conduct thorough assessments during 20–40 minute visits, ensuring high-quality patient interactions
- Collaborate with an integrated care team, including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, laboratory and pharmacy staff, and referral coordinators
- Utilize electronic medical records (EMR) for efficient and accurate documentation
- Participate in value-based care initiatives, focusing on improved patient outcomes and healthcare quality
- Engage in ongoing training and professional development, including a comprehensive onboarding program covering value-based care, documentation best practices, EMR optimization, and integrated care workflows
- Maintain compliance with all regulatory and quality standards relevant to primary care delivery

Qualifications & Requirements
- MD or DO degree required
- Board certification or eligibility in Internal Medicine or Family Medicine
- Geriatric experience preferred but not required
- Experience with HEDIS measures and value-based care is a plus
- Ability to work collaboratively in a multidisciplinary care team
- Excellent communication and interpersonal skills
