As a full-time Primary Care Physician (MD/DO), you will work within a supportive, integrated care team environment. You will focus on providing comprehensive care to a predominantly Medicare Advantage patient base, emphasizing chronic disease management, preventive services, and value-based outcomes. Primary Care Physician (MD/DO) - Summary
- Deliver evidence-based, compassionate primary care to adults, primarily aged 65+, within a value-based care model.
- Practice within a multidisciplinary team to optimize patient outcomes and satisfaction.

Duties & Responsibilities
- Provide comprehensive, longitudinal primary care to a panel of adult patients, with a focus on prevention and chronic disease management.
- Manage a patient panel averaging 12–15 patients per day, allowing for more personalized care and thorough visits.
- Lead and collaborate with an integrated care team, including nurse practitioners (NPs), physician assistants (PAs), medical assistants, social workers, laboratory professionals, and pharmacy support staff.
- Coordinate care with specialists and facilitate referrals as needed while maintaining continuity of care.
- Engage in quality improvement initiatives and utilize clinical metrics (such as HEDIS) to drive better patient outcomes.
- Participate in training and ongoing education related to value-based care and population health management.
- Document all patient encounters and clinical activities in compliance with regulatory requirements and organizational policies.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board certified or board eligible in Internal Medicine or Family Medicine.
- Geriatric care experience preferred but not required.
- Familiarity with HEDIS measures is a plus.
- Previous Value-Based Care (VBC) experience is desirable; willingness to learn and participate in training is welcomed.
- Active and unrestricted medical license in the state of Florida or eligibility to obtain licensure.
