Primary Care Physician (PCP) – Summary
- Provide primary care to local Medicare populations in a value-based, patient-centered care model.
- Work full-time with a dedicated care team in a supportive and collaborative clinical environment.
- Utilize advanced electronic medical record systems (Canopy and Greenway) to deliver high-quality care.

Duties & Responsibilities
- Deliver comprehensive primary care services to a diverse patient population, focusing on prevention, chronic disease management, and wellness.
- Manage a daily patient volume of 13-16 individuals, with 20-minute regular visits and extended 40-minute visits for new or complex cases.
- Collaborate with an integrated care team, including scribes, medical assistants, and social workers, to coordinate patient care and support optimal health outcomes.
- Leverage onsite laboratory services and access to specialists through RubiconMD for enhanced patient management.
- Utilize electronic medical records (EMR) for accurate documentation and efficient workflow.
- Participate in community patient activities, such as exercise classes, wellness events, and educational workshops, fostering patient engagement and holistic wellness.
- Adhere to a value-based care approach aimed at reducing hospital admissions and improving overall patient satisfaction.
- Collaborate with a team of 3-5 providers per clinic, including nurse practitioners and other primary care physicians.
Salary & Benefits
- Competitive base salary in the range of $230,000-$250,000, with additional incentive bonuses based on quality metrics and patient outcomes.

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree required.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Fellowship training in Geriatrics is preferred but not required.
- Residency completion is required; this position does not accept current residents.
