Primary Care Nurse Practitioner (NP) / Physician Assistant (PA) - Summary
- Permanent, full-time opportunity for a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA).
- Collaborate with a multidisciplinary team to deliver comprehensive primary care services.
- Provide patient-centered care across a broad spectrum of health needs, including preventive, acute, and chronic conditions.
Duties & Responsibilities
- Conduct thorough patient assessments, including histories and physical examinations, for individuals of all ages.
- Order and interpret diagnostic tests, such as laboratory work, imaging, and screenings, to inform treatment plans.
- Diagnose and manage both acute and chronic medical conditions using evidence-based practices.
- Develop, implement, and monitor individualized care plans in collaboration with patients, families, and interdisciplinary teams.
- Prescribe medications and other therapies as appropriate within the scope of licensure.
- Educate patients and families on preventive health measures, disease management, and healthy lifestyles.
- Refer patients to specialists and coordinate care as needed to ensure comprehensive treatment.
- Maintain accurate and detailed electronic health records in compliance with regulatory standards.
- Participate in quality improvement initiatives and contribute to the development of best practices in primary care.
- Adhere to all legal, ethical, and professional standards in patient care, documentation, and confidentiality.

Qualifications & Requirements
- Current, unrestricted licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in good standing.
- Graduate of an accredited Nurse Practitioner or Physician Assistant program.
- National certification as an NP or PA (e.g., AANP, ANCC, NCCPA).
- Current Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) preferred.
- Experience in primary care, family medicine, or internal medicine preferred but not required.
- Excellent clinical, diagnostic, and communication skills.
- Ability to work effectively in a collaborative, team-based environment.
