Primary Care Nurse Practitioner (NP) / Physician Assistant (PA) - Summary
- Deliver high-quality, evidence-based primary care to patients of all ages
- Work collaboratively with an interdisciplinary healthcare team to develop and implement care plans
- Emphasize preventive care, chronic disease management, and acute illness treatment
- Foster strong patient-provider relationships built on trust and communication
- Engage in continuous professional development and support organizational quality initiatives
Duties & Responsibilities
- Conduct comprehensive patient assessments, including history-taking, physical examinations, and diagnostic testing
- Diagnose and treat a variety of acute and chronic conditions commonly seen in primary care
- Order and interpret laboratory and imaging studies as appropriate
- Develop, implement, and monitor individualized treatment plans
- Prescribe medications and other therapies within the scope of practice
- Provide patient education and counseling on health promotion, disease prevention, and lifestyle modifications
- Coordinate referrals to specialists and community resources as needed
- Document all patient encounters clearly and accurately in the electronic health record
- Participate in team meetings, case conferences, and quality improvement projects
- Adhere to all clinical protocols, policies, and regulatory standards

Qualifications & Requirements
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current, unrestricted state licensure as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Certification by an accredited board (e.g., AANP, ANCC, NCCPA)
- Current BLS certification (or ability to obtain prior to start date)
- Strong clinical assessment and diagnostic skills
- Excellent communication, interpersonal, and organizational abilities
- Commitment to evidence-based practice and quality improvement
- Experience in primary care or family medicine preferred, but not required
