Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) - Summary
- Permanent, full-time position within a collaborative primary care setting

Duties & Responsibilities
- Conduct comprehensive patient assessments and physical examinations
- Diagnose and manage acute and chronic medical conditions
- Develop, implement, and monitor individualized care plans
- Order, interpret, and evaluate diagnostic tests and laboratory results
- Prescribe medications and non-pharmacologic treatments in accordance with current guidelines
- Educate patients and families about health maintenance, disease prevention, and treatment plans
- Coordinate care with physicians, specialists, and other healthcare professionals
- Document all patient encounters in the electronic medical record system
- Participate in quality improvement initiatives and continuing education activities
- Ensure compliance with all federal, state, and organizational regulations

Qualifications & Requirements
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Certification by an accredited NP or PA certifying body
- Graduation from an accredited Nurse Practitioner or Physician Assistant program
- Experience in primary care or a closely related clinical field preferred
- Current Basic Life Support (BLS) certification
- Strong clinical assessment and diagnostic skills
- Excellent written and verbal communication abilities
