Duties & Responsibilities
- Provide comprehensive primary care services to patients in an outpatient setting
- Conduct patient assessments, diagnose conditions, develop treatment plans, and manage ongoing care
- Promote preventive care and educate patients on health maintenance and disease prevention
- Work collaboratively with physicians, nurses, and support staff to deliver coordinated care
- Utilize a robust EMR (EPIC) for efficient documentation and care delivery
- Participate in minimal phone-only call rotation (approximately 1 in 6 weekends)
- Refer patients to subspecialists as needed and coordinate follow-up care
- Engage in quality improvement initiatives and community health promotion

Qualifications & Requirements
- Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Valid or eligible for state licensure
- Experience in family or adult primary care preferred
