Post Acute Care Physician – SNF, Primary Care, MD/DO Summary- Full-time position providing direct primary care to senior residents in a skilled nursing facility.

- Board certification in internal medicine or family medicine required (MD or DO).
- Experience with Medicare Advantage patient populations preferred.

- Provide comprehensive primary care services to a daily census of 12-18 senior patients in a skilled nursing facility setting.
- Conduct thorough patient assessments, develop individualized care plans, and oversee ongoing management of chronic and acute conditions.
- Perform standard primary care procedures and coordinate care with interdisciplinary team members, including nursing and support staff.
- Implement clinical protocols designed to minimize emergency room visits and prevent unnecessary hospital admissions.
- Ensure compliance with all regulatory and documentation standards, maintaining accurate and timely medical records for all patients.
- Collaborate with families, caregivers, and specialists to support patients’ holistic health and well-being.
- Participate in regular team meetings and quality improvement initiatives within the skilled nursing facility setting.
- Maintain open communication with facility administration and participate in ongoing staff development and training.

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board certification in Internal Medicine (ABIM) or Family Medicine (ABFM) is required.
- Active state medical license in good standing.
- Active DEA registration in the state, or willingness to transfer DEA credentials as required.
- Basic Life Support (BLS) certification.
- Prior experience working with senior populations and/or Medicare Advantage programs is preferred.
- Demonstrated commitment to patient-centered care and interdisciplinary collaboration.
- Board-certified physician (MD or DO) with a passion for geriatric medicine and post-acute care.
- Experience providing primary care in skilled nursing facilities or similar senior care environments.
- Ability to effectively manage a moderate daily patient census while ensuring high-quality, individualized care.
- Strong communication and collaboration skills for working with support staff, families, and interdisciplinary teams.
- Detail-oriented with a commitment to regulatory compliance and excellence in documentation.
- Adaptable, compassionate, and dedicated to improving outcomes for senior patients.
- Shifts are typically Monday through Friday, 8:00 AM to 5:00 PM, with no weekend requirements specified.
