Post-Acute Care Physician (SNF) – MD/DO Summary
- Serve as a post-acute care physician in a skilled nursing facility
- Full-time, onsite locums position with a locums-to-permanent track

Duties & Responsibilities
- Provide direct patient care to residents in a skilled nursing facility
- Diagnose, manage, and treat a full spectrum of medical conditions common to post-acute and geriatric populations
- Work collaboratively with facility staff, including nurses, therapists, and APPs
- Oversee and supervise advanced practice providers as required
- Participate in interdisciplinary team meetings and care planning
- Document clinical encounters using facility EMR systems
- Engage in flexible call coverage with additional compensation opportunities
- Maintain high standards for clinical excellence and patient safety
- Participate in leadership and supervisory opportunities as available

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board-Certified or Board-Eligible in Internal Medicine or Family Medicine
- Eligible for medical licensure in the state of Colorado
- Experience in geriatrics, senior care, or post-acute settings preferred
- Demonstrated ability to work effectively with interdisciplinary teams
- Comfortable supervising and collaborating with APPs
