As a Post-Acute Care Physician (SNF, Locum Tenens), you will work closely with a multidisciplinary team, including advanced practice providers (APPs), to deliver high-quality care to patients with diverse medical needs in the post-acute setting. This is a full-time, onsite locum tenens role with an attractive compensation structure, comprehensive malpractice coverage, and all travel and lodging expenses paid. Post-Acute Care Physician (SNF, Locum Tenens) - Internal Medicine/Family Medicine - Summary- Provide comprehensive post-acute care in skilled nursing facility settings
- Work as a locum tenens physician with the potential for permanent employment
- Collaborate with a multidisciplinary team to deliver patient-centered care
- Participate in a robust network with opportunities for leadership and supervision
- Conduct initial and ongoing patient assessments in the SNF environment
- Develop and implement individualized care plans for post-acute and geriatric patients
- Supervise and collaborate with Advanced Practice Providers (APPs) as part of the care team
- Engage in interdisciplinary care coordination with nursing staff, therapists, and other providers
- Maintain accurate and timely documentation of patient encounters and care transitions
- Participate in flexible call coverage with additional compensation provided
- Actively contribute to quality improvement initiatives and patient safety programs
- Support and mentor junior clinicians and participate in leadership opportunities as available
- $150/hr for locum tenens assignment
- Potential for permanent employment with a base salary plus uncapped wRVU structure

- MD or DO degree from an accredited medical school
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active and unrestricted medical license
- Experience in geriatrics, senior care, or post-acute medicine preferred
- Comfort with supervising and collaborating with APPs
- Strong interpersonal skills and commitment to high-quality, patient-centered care
- Board-certified or board-eligible in Internal Medicine or Family Medicine
- Clinical experience in post-acute, SNF, or geriatric settings is highly desirable
