Post-Acute Care Physician (MD/DO) – Full-Time – Greater Los Angeles, CA
We are seeking a dedicated and experienced Post-Acute Care Physician (MD/DO) to join our value-driven healthcare organization serving skilled nursing facilities (SNFs) throughout the Greater Los Angeles, CA area. In this full-time position, you will play a pivotal role in improving the quality of patient care for seniors and medically complex individuals transitioning from hospital to post-acute settings. As a Post-Acute Care Physician, you will assume responsibility for comprehensive medical management of post-acute patients located in partnered SNFs. You will collaborate closely with nurse practitioners, registered nurses, and facility teams in a multidisciplinary environment, empowering you to deliver high-quality, coordinated care. - Conduct daily rounds in partnered skilled nursing facilities (SNFs)
- Deliver comprehensive medical management and chronic disease care

- Active California medical license
