Deliver the right care, at the right time, in the right place as a compassionate and skilled Physical Therapist. Provide home-based physical therapy services to patients in Dekalb and Olga Counties. Responsibilities include assessing patient functional mobility, developing individualized care plans, and implementing evidence-based interventions to support recovery and independence. Collaborate with an interdisciplinary healthcare team to optimize outcomes and ensure patient-centered care. Maintain accurate documentation and comply with all regulatory and company standards.
Qualifications:
- Active Physical Therapist (PT) license in the state of Illinois
- CPR certification (AHA or American Red Cross)
- Minimum 1 year of experience in a healthcare setting
- Ability to meet physical demands of home care environment
- Reliable transportation
- Strong communication and interpersonal skills
