How You'll Make an Impact
- Deliver high-quality, patient-centered physical therapy in the home
- Evaluate functional needs and develop customized plans of care
- Collaborate with physicians, caregivers, and interdisciplinary teams
- Adjust care plans to optimize patient independence and outcomes
- Lead and mentor support staff toward patient success
- Maintain accurate, timely documentation and compliance standards
- Promote physical therapy excellence within the community
What Youll Need to Succeed
- Graduate of an APTA-approved Physical Therapy program
- Current, unrestricted PT license in the state of practice
- Minimum 1 year of experience in a healthcare setting
- Ability and willingness to travel up to 50% within the coverage area
- Valid drivers license & auto insurance
- Ability to lift 50100 lbs and perform physical job demands
