Responsibilities:
- Promotes Elara Carings philosophy, mission statement, and administrative policies to ensure quality of care.
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Designs and implements a plan of care for patients based on assessments and involves significant others when appropriate.
- Conducts therapy practice within defined standards of care.
- Directs support staff to achieve patients functional goals.
- Documents evaluations, treatment goals, and care plans and maintains prescriptions and signatures.
- Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
Qualifications:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare facility.
- Able and willing to travel within branch/office coverage area (up to 50%).
- Valid drivers license, dependable vehicle, and current auto insurance.
- Able to lift 50-100 lbs and perform physically demanding tasks.
