Job Description:
At Elara Caring, Physical Therapists provide high-quality care to patients in their homes, evaluating functional needs and outcomes, designing and implementing care plans, and consulting with other specialists to ensure optimal patient results.
- Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Designs and implements a plan of care for patients based on assessments.
- Conducts therapy practice within defined standards of care.
- Directs the assistance of support staff to achieve patients functional goals.
- Documents evaluations, treatment goals, and plans, maintaining prescriptions and signatures.
- Provides direction and leadership to staff and acts as a resource to promote physical therapy in the community.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare facility.
- Able and willing to travel within branch/office coverage area (up to 50%).
- Dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
