Physical Therapist (PT) Home Health - Summary- Deliver in-home physical therapy services to patients recovering from illness, surgery, or injury
- Collaborate with interdisciplinary teams to create and implement individualized care plans
- Promote the mission and values of Elara Caring through high-quality, compassionate care
- Support patients in achieving maximum functional independence in their own home environment
- Conduct comprehensive patient assessments to determine mobility, strength, and functional limitations
- Develop goal-oriented, personalized treatment plans in accordance with physician orders and best practices
- Deliver direct patient care including therapeutic exercises, functional training, and manual therapy
- Monitor patient progress and adjust care plans as needed to optimize outcomes
- Maintain up-to-date documentation, including progress notes, treatment plans, and required signatures
- Provide guidance and leadership to physical therapy assistants and support staff
- Participate in interdisciplinary case conferences and collaborate with healthcare professionals
- Educate patients and families on exercises and techniques to continue progress between visits
- Advocate for patient needs and participate in community outreach to promote physical therapy services

- Current, unrestricted Physical Therapist (PT) license valid for the state of practice
- Minimum of one (1) year of experience in a healthcare setting preferred
- Valid driver’s license, current auto insurance, and reliable vehicle for travel to patient homes
- Ability to travel within the designated branch or office coverage area, as required
- Physical ability to lift, push, pull, and support up to 50 pounds
- Strong interpersonal, communication, and organizational skills
