Physical Therapist (PT) – Home Health Summary- Deliver comprehensive physical therapy services to patients in a home health setting.
- Work collaboratively with an interdisciplinary team to provide individualized patient care.
- Promote patient safety, mobility, and independence through evidence-based practice.
- Ensure all care is provided in line with Elara Caring’s mission and standards.
- Conduct thorough initial and ongoing patient assessments to determine physical therapy needs.
- Develop and implement customized treatment plans based on physician orders and patient goals.
- Deliver therapeutic interventions such as exercise programs, manual therapy, and patient education.
- Monitor patient progress, adjust treatment plans, and communicate updates to the healthcare team.
- Document all care activities, assessments, and interventions accurately and timely according to policy.
- Educate patients and caregivers on techniques to enhance mobility, prevent injury, and support recovery at home.
- Collaborate with nurses, physicians, and other allied health professionals to coordinate comprehensive care.

- Current, active Physical Therapist (PT) license as required by state regulations.
- Valid CPR certification from the American Heart Association or American Red Cross.
- Minimum of one year of experience in a healthcare facility, preferably in home health or rehabilitation.
- Ability to meet the physical demands of the role, including lifting, transferring, and assisting patients as needed.
- Reliable transportation to perform in-home patient visits as required.
