Physical Therapist (PT) Home Health - Summary
- Deliver personalized physical therapy services to patients in their homes
- Work collaboratively with healthcare team members to ensure coordinated and effective care

Duties & Responsibilities
- Conduct comprehensive assessments of patients' physical abilities and limitations
- Develop, implement, and monitor individualized care plans tailored to patient needs
- Administer physical therapy treatments, including exercises, manual therapy, and adaptive equipment instruction
- Educate patients and family members about rehabilitation techniques and safety measures
- Document patient progress, treatment plans, and outcomes accurately and in a timely manner
- Communicate effectively with physicians, nurses, and other allied health professionals
- Adjust care plans based on ongoing evaluations and patient progress
- Ensure compliance with all regulatory, accreditation, and company standards
- Maintain confidentiality of patient information in accordance with HIPAA and organizational policies
- Provide recommendations for adaptive equipment and home modifications when necessary

Qualifications & Requirements
- Current and valid Physical Therapist (PT) license
- CPR certification from the American Heart Association or American Red Cross
- Minimum of one year of experience in a healthcare facility, preferably in home health or a related setting
- Ability to meet the physical demands required for in-home patient care
- Reliable transportation and willingness to travel to patient homes as needed
- Strong verbal and written communication skills
