- Provide high-quality, evidence-based rehabilitative care to patients in their homes as a Physical Therapist (PT) Home Health.

Duties & Responsibilities

- Design, implement, and regularly update individualized plans of care, incorporating input from patients, families, and significant others as appropriate.
- Maintain accurate and timely documentation of evaluations, treatment goals, progress notes, and care plans, ensuring compliance with regulatory and organizational standards.
- Collaborate and consult with physicians, nurses, and other specialists to optimize patient outcomes.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- Minimum of one (1) year of clinical experience in a healthcare facility, preferably with home health or rehabilitative focus.
- Ability and willingness to travel within the assigned branch coverage area; must possess a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds and meet the physical demands of the position.
