Physical Therapist (PT) Home Health - Summary- Provide high-quality, evidence-based rehabilitative care to patients in their homes as a Physical Therapist (PT) Home Health.
- Participate in interdisciplinary team meetings and case conferences to ensure coordinated patient care.

- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- Minimum of one (1) year of clinical experience in a healthcare facility, preferably with home health or rehabilitative focus.
- Ability and willingness to travel within the assigned branch coverage area; must possess a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Physical ability to frequently lift, push, pull, and support up to 50 pounds and meet the physical demands of the position.
