Physical Therapist (PT) Home Health - Summary- Deliver physical therapy services to patients in their homes, helping them restore function, improve mobility, and achieve their highest level of independence.
- Work collaboratively with an interdisciplinary healthcare team to ensure patient-centered care.
- Serve a diverse patient population, bringing compassionate care and clinical expertise to every visit.
- Participate in ongoing professional development and benefit from mentorship opportunities.
- Conduct comprehensive patient assessments to determine therapy needs and develop individualized care plans.
- Implement evidence-based physical therapy interventions to address patient goals and functional limitations.
