Responsibilities:
- Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Designs and implements a plan of care for patients based on a thorough assessment and incorporates participation by significant others when appropriate.
- Conducts therapy practice within defined standards of care.
- Directs support staff to achieve patients functional goals.
- Documents evaluations, treatment goals, and plans while maintaining required prescriptions and signatures.
- Provides leadership and acts as a resource to promote physical therapy in the community.
- Handles a physically demanding environment, requiring lifting and transfer of patients.
- May work irregular or on-call hours as needed.
Qualifications:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare facility.
- Able and willing to travel within branch/office coverage area (up to 50%).
- Must have a dependable vehicle, valid drivers license, and auto insurance in accordance with state laws.
- Able to sit, stand, bend, lift and move intermittently and lift 50-100 lbs.
