Responsibilities:
- Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists to ensure optimal patient outcomes.
- Designs and implements individualized care plans based on thorough patient assessments and family participation.
- Conducts therapy practice within defined standards of care and directs support staff to achieve patients functional goals.
- Documents evaluations, treatment goals, plans, and updates documentation as necessary.
- Provides direction and leadership to staff and acts as a resource promoting physical therapy in the community.
Qualifications:
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted PT license valid for the state of work.
- Minimum one (1) year of experience in a healthcare setting.
- Able and willing to travel within branch/office coverage area (up to 50%).
- Must have a dependable vehicle, valid drivers license, and current auto insurance in accordance with state laws.
