Physical Therapist (PT) Home Health - Summary
The Physical Therapist (PT) Home Health role at Elara Caring centers on delivering evidence-based physical therapy services to patients in their homes. You will assess, plan, and implement individualized care programs designed to improve patient function, mobility, and overall quality of life. Duties & Responsibilities
- Promote Elara Caring’s mission, philosophy, and administrative policies to ensure delivery of quality home health care.
- Provide direct patient care, including thorough assessments, development of treatment plans, and evaluation of outcomes.
- Consult and collaborate with other healthcare professionals to optimize patient outcomes and adjust care plans as necessary.
- Design and implement care plans based on comprehensive assessments, incorporating input from patients and their families when appropriate.
- Conduct all physical therapy practices within defined standards and regulatory guidelines.
- Provide direction and leadership to support staff to help patients achieve their functional goals.
- Document evaluations, treatment goals, and progress accurately, maintaining compliance with all required documentation standards.
- Serve as a resource and role model for physical therapy within the community.
- Work in physically demanding environments, including handling, lifting, and transferring patients as needed.
- Potentially respond to irregular work hours or on-call demands as necessary.

Qualifications & Requirements
- Graduate of an accredited American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of practice.
- Minimum of one year of experience in a healthcare facility preferred.
- Ability and willingness to travel within the assigned coverage area to provide home health visits.
- Dependable vehicle, valid driver’s license, and current auto insurance as required by state law.
- Physical capability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning and equipment movement.
