Physical Therapist (PT) – Home Health Summary
As a Physical Therapist (PT) – Home Health, you will provide direct patient care, evaluate functional needs, and develop comprehensive care plans for individuals receiving services in their homes. Your expertise will help patients regain mobility, improve strength, and enhance their quality of life. Working collaboratively with other healthcare professionals, you will ensure each patient receives individualized, evidence-based care tailored to their unique needs. Duties & Responsibilities
- Promote the organization’s philosophy, mission, and policies by consistently providing high-quality care to patients in their homes.
- Conduct thorough assessments of patients’ functional abilities and limitations to develop appropriate, individualized plans of care.
- Deliver direct physical therapy interventions aimed at improving patients’ mobility, strength, balance, and independence.
- Collaborate with patients, caregivers, and multidisciplinary teams to set and achieve measurable therapy goals.
- Continuously evaluate patient progress and adjust treatment plans as needed to ensure optimal outcomes.
- Maintain accurate and timely documentation of evaluations, treatment plans, progress notes, and other required records.
- Provide guidance and leadership to support staff, fostering a culture of excellence and collaboration within the home health team.
- Educate patients and their families on exercises, safety, and strategies for improving function and quality of life at home.
- Ensure all care provided is in accordance with organizational protocols, professional standards, and regulatory requirements.
- May be required to work in physically demanding environments, including performing transfers, lifting, and assisting patients as necessary.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment.
- Minimum of one year of experience in a healthcare facility, preferably in home health or a related field.
- Able and willing to travel within the assigned coverage area as required for patient care.
- Reliable transportation, valid driver’s license, and current auto insurance in accordance with state laws.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including patient transfers and moving equipment as needed.
- Demonstrated commitment to high standards of clinical practice, patient safety, and ethical conduct.
