As a Physical Therapist (PT) Home Health specialist, you will be responsible for the assessment, design, and implementation of individualized treatment plans for patients requiring physical therapy in their home environments. Your primary goal will be to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. You will play a vital role in supporting patients throughout their health journey by providing direct care, educating family members, and collaborating with other healthcare professionals to ensure continuity and excellence in care delivery.

Duties & Responsibilities
- Provide direct patient care, assessing functional needs and developing comprehensive care plans tailored to individual patient goals.
- Promote and uphold Elara Caring’s philosophy, mission statement, and administrative policies to ensure the highest quality of care.
- Evaluate patient progress and adjust therapy plans as needed to achieve optimal outcomes.
- Design interventions based on thorough assessments and active patient/family participation.
- Document evaluations, treatment goals, progress notes, and maintain accurate clinical records in compliance with organizational and regulatory standards.
- Direct and supervise support staff in the implementation of therapy interventions to help patients achieve functional independence.
- Serve as a resource and leader within the community, promoting the benefits of physical therapy in home health settings.
- Maintain a high standard of care in physically demanding and dynamic environments, including handling, lifting, and transferring patients.
- Respond flexibly to irregular hours, including potential on-call responsibilities as required by patient needs.

Qualifications & Requirements
- Graduation from an American Physical Therapy Association (APTA) approved physical therapy program.
- Current, unrestricted Physical Therapist (PT) license valid for practice in the state of employment.
- Minimum of one year of experience in a healthcare setting is preferred.
- Ability and willingness to travel within the assigned branch or office coverage area; must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Physical capacity to frequently lift, push, pull, and support up to 50 pounds, including patient positioning, transferring, and equipment movement.
