Physical Therapist (PT) Home Health - Summary
- Provide comprehensive in-home physical therapy services to patients, focusing on individualized, evidence-based care.
- Travel to multiple patient residences to deliver therapy interventions and follow-up care.

Duties & Responsibilities
- Conduct thorough patient assessments to identify functional limitations, mobility challenges, and rehabilitation goals.
- Develop, implement, and revise individualized Plans of Care in partnership with patients and their families.
- Deliver hands-on physical therapy interventions, monitor patient progress, and adjust treatment plans accordingly.
- Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to ensure integrated, patient-centered care.
- Document evaluations, treatments, and patient responses accurately and in a timely manner, ensuring compliance with clinical and regulatory standards.
- Educate patients and caregivers on exercises, mobility strategies, and safety precautions to promote independence.
- Participate in ongoing professional development and training to maintain clinical competencies and advance your practice.
- Provide mentorship and support to therapy assistants and other team members as needed.

Qualifications & Requirements
- Graduate of an APTA-accredited Physical Therapy program.
- Minimum of one (1) year experience as a Physical Therapist.
- Current, unrestricted Physical Therapist (PT) license for all assigned travel regions.
- Willingness and ability to travel extensively to patient locations as needed.
- Reliable transportation, a valid driver’s license, and current auto insurance.
- Ability to safely lift and transfer up to 100 pounds as required for patient care.
- Strong communication, organizational, and documentation skills.
