Physical Therapist (PT) Home Health - Summary- Provide high-quality, patient-centered physical therapy services in a home health environment
- Work collaboratively with patients, families, and interdisciplinary healthcare teams
- Support the mission and values of Elara Caring
- Deliver direct physical therapy care to patients in their homes
- Conduct comprehensive evaluations to determine functional needs and set measurable goals
- Develop, implement, and adjust individualized treatment plans based on clinical assessments and patient progress
- Document evaluations, treatment plans, goals, and progress notes in accordance with company and regulatory standards
- Collaborate with physicians, nurses, and other allied health professionals to coordinate patient care
- Educate patients and caregivers on therapy techniques, home exercise programs, and safety measures
- Supervise and direct support staff as appropriate to achieve patients’ functional goals
- Provide leadership and serve as a resource for physical therapy education and awareness within the community
- Adhere to established standards of care and company policies at all times
- Participate in quality improvement initiatives and ongoing professional development
- Assist with coverage of irregular hours or on-call responsibilities as needed

- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program
- Current, unrestricted Physical Therapist (PT) license valid in the state of practice
- Minimum of one (1) year experience in a healthcare facility, preferably with home health or rehabilitation experience
- Ability and willingness to travel within the assigned coverage area as needed
- Reliable transportation, valid driver’s license, and current auto insurance compliant with state laws
- Ability to lift, push, pull, and support up to 50 pounds as part of daily responsibilities
- Strong organizational, communication, and documentation skills
