Responsibilities:
- Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
- Designs and implements individualized plans of care based on thorough assessments and patient participation.
- Documents evaluations, treatment goals, and progress updates, maintaining accurate records and prescriptions.
- Directs support staff to achieve patient functional goals and promotes physical therapy awareness in the community.
- Promotes Elara Carings philosophy, mission, and policies to ensure quality of care.
Requirements:
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current unrestricted Physical Therapist license in the state of practice.
- Minimum of one (1) year of experience in a healthcare facility.
- Dependable vehicle, valid drivers license, and current automobile insurance.
- Able and willing to travel within the coverage area (up to 50%).
- Ability to frequently lift, push, pull, or support up to 50 pounds.
