Job Description:
At Elara Caring, Physical Therapists provide care in patients' homes, focusing on quality outcomes and individualized plans. Responsibilities include:
- Providing direct patient care, evaluating functional needs and outcomes, and consulting with specialists.
- Designing and implementing care plans based on assessments, involving family or caregivers as appropriate.
- Conducting therapy practice within defined standards of care.
- Directing support staff to achieve patients functional goals.
- Documenting evaluations, treatment goals, plans, and updates.
- Serving as a resource to promote physical therapy in the community.
Qualifications:
- Graduate of an APTA-approved physical therapy program.
- Current, unrestricted PT license in the state of practice.
- Minimum one year of experience in a healthcare facility.
- Ability and willingness to travel within branch coverage area (up to 50%).
- Valid drivers license and auto insurance, dependable vehicle.
- Ability to lift, push, pull, and support up to 50 pounds, including transferring patients and equipment.
