Physical Therapist (PT) Home Health - Summary
- Provide high-quality, patient-centered physical therapy services in a home health setting.
- Evaluate patient needs, establish care plans, and work collaboratively with interdisciplinary teams.

Duties & Responsibilities
- Promote Elara Caring’s philosophy, mission, and administrative policies to ensure the delivery of quality care.
- Deliver direct patient care through thorough assessments, functional evaluations, and outcome measurements.
- Develop and implement personalized plans of care tailored to each patient’s needs and goals.
- Conduct therapy practices within defined standards of care and industry best practices.
- Supervise and provide direction to support staff to maximize patient progress towards functional goals.
- Accurately document evaluations, treatment goals, plans of care, and maintain necessary prescriptions and signatures.
- Lead and mentor staff, fostering a culture of excellence and promoting physical therapy within the community.
- Adapt to flexible scheduling, which may include irregular hours and on-call duties when necessary.

Qualifications & Requirements
- Graduate of a physical therapy program accredited by the American Physical Therapy Association (APTA).
- Possess a current, unrestricted Physical Therapist (PT) license valid in the state of practice.
- At least one year of experience in a healthcare facility, preferably with exposure to home health or similar settings.
- Ability and willingness to travel within the designated branch or office coverage area (up to 50%).
- Dependable vehicle, valid driver’s license, and current auto insurance required.
- Physical capability to frequently lift, push, pull, and support up to 50 pounds as part of patient care.
