Physical Therapist (PT) - Home Health Summary- Deliver home-based physical therapy services to patients as part of a collaborative healthcare team.
- Develop and implement individualized care plans based on clinical assessments.

- Design and execute customized treatment plans to maximize patient mobility and independence.
- Consult with physicians and other healthcare professionals to coordinate patient care.
- Supervise and direct support staff to achieve patient-specific functional goals.
- Document patient evaluations, treatment goals, progress notes, and maintain accurate records in compliance with healthcare standards.
- Promote physical therapy services within the community and act as a resource for both patients and staff.

- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current, valid, and unrestricted Physical Therapist (PT) license in the state of practice.
- Minimum of one (1) year of experience in a healthcare facility (hospital, clinic, or home health setting).
- Demonstrated ability to travel within the designated branch or office coverage area as required.
- Reliable transportation, a valid driver's license, and current auto insurance in accordance with state regulations.
- Strong clinical judgment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Demonstrated commitment to patient-centered care and rehabilitation excellence.
- Ability to work independently and as part of an interdisciplinary team.
