Duties & Responsibilities
- Provide direct, evidence-based physical therapy care to patients in home health settings.
- Perform comprehensive functional assessments to identify patient needs and establish measurable goals.
- Develop, execute, and continuously update individualized treatment plans.
- Document all evaluations, interventions, and progress notes accurately and in a timely manner according to company policies and regulatory requirements.
- Participate in interdisciplinary team meetings to coordinate care and ensure a holistic approach to patient recovery.
- Educate patients and their families on care plans, therapeutic exercises, use of equipment, and strategies to maximize independence and safety at home.

Qualifications & Requirements
- Graduate of an accredited physical therapy program recognized by the American Physical Therapy Association (APTA).
- Current, unrestricted Physical Therapist (PT) license valid for the state of practice.
- At least one (1) year of experience in a healthcare facility, preferably with exposure to home health or similar environments.
- Valid driver’s license, reliable transportation, and current auto insurance, as required by state regulations.
- Ability to travel within the designated branch coverage area as needed (up to 50%).
- Physical capacity to lift, push, pull, and support up to 50 pounds, including assisting patients with mobility and transfers.
- Excellent communication, interpersonal, and organizational skills.
