- Provide physical therapy services to patients in their homes, focusing on individualized care plans and functional improvement.

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited physical therapy program.
- Current and unrestricted Physical Therapist (PT) license in the state of practice.
- At least one year of experience in a healthcare facility, preferably with exposure to home health settings.
- Ability and willingness to travel within the designated coverage area as required for patient visits.
- Valid driver’s license, reliable vehicle, and up-to-date auto insurance.
- Physical capability to lift, push, or pull up to 50 pounds as part of job duties.
