Physical Therapist (PT) – Home Health Summary- Deliver in-home outpatient rehabilitation services under Medicare Part A
- Support post-acute, homebound senior patients with skilled therapy interventions
- Collaborate within a dedicated home health agency focused on patient-centered care
- Full-time, flexible work schedule to meet patient visit requirements
- Conduct comprehensive physical therapy assessments for homebound patients
- Develop and implement individualized treatment plans to address mobility, strength, and functional goals
- Provide skilled physical therapy interventions, including therapeutic exercises, gait training, and pain management
- Document patient progress and outcomes in compliance with Medicare Part A regulations
- Communicate effectively with patients, families, and interdisciplinary healthcare team members
- Educate patients and caregivers on safe mobility techniques and home exercise programs
- Travel between patient homes within the service area as needed
- Maintain accurate, timely clinical records and ensure adherence to agency protocols and documentation standards

- Doctor of Physical Therapy (DPT) or equivalent degree preferred; candidates with extensive homecare experience also considered
- Minimum of 5 years of clinical experience, with home health experience strongly preferred
- Current physical therapy licensure in accordance with state requirements
- Expertise in outpatient rehabilitation and Medicare Part A documentation
- Ability to manage a flexible schedule and meet weekly visit equivalents (27 per week for Part A therapists)
- Strong communication, organizational, and clinical skills
- Reliable transportation to travel within the service area
- Experienced Physical Therapist (PT) with a passion for home health and senior care
- Demonstrates clinical proficiency in post-acute rehabilitation and skilled therapy services
- Comfortable managing higher-level patient care and detailed documentation
- Adaptable, collaborative, and committed to delivering patient-centered care
