Physical Therapist (PT), Home Health - Summary
- Deliver in-home physical therapy services for homebound patients under Medicare Part A.
- Support patients recovering from acute events, such as surgeries or injuries, to regain strength, mobility, and independence.
- Collaborate with interdisciplinary teams to provide holistic patient care and promote overall well-being.
- Full-time position with flexible scheduling based on patient visit requirements.
Duties & Responsibilities
- Conduct thorough physical therapy assessments for homebound adult patients, primarily seniors, to determine care needs and develop individualized treatment plans.
- Implement evidence-based physical therapy interventions to help patients increase strength, improve range of motion, and achieve rehabilitation goals.
- Document all patient interactions, assessments, and progress notes in compliance with Medicare Part A documentation standards.
- Manage a caseload averaging 27 visit equivalents per week, including starts of care and regular follow-up visits.
- Coordinate care delivery with healthcare team members, including physicians, nurses, and social workers, to ensure comprehensive and effective patient care.
- Educate patients and their caregivers on therapeutic exercises, safety, and techniques to maintain progress between visits.
- Travel within the Greenwich, CT area and nearby communities to provide in-home rehabilitation services.
- Maintain up-to-date knowledge of best practices in home health and outpatient rehabilitation.

Qualifications & Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent education. DRPH preferred but not required.
- Current and valid Physical Therapist (PT) license.
- Five or more years of clinical experience in physical therapy, with a preference for home health experience.
- Thorough knowledge of Medicare Part A standards and the ability to complete detailed clinical documentation.
- Strong organizational and time-management skills to manage patient caseload and documentation efficiently.
- Reliable transportation for travel within the Greenwich, CT area and neighboring communities.
