Physical Therapist (PT), Home Health – Full-time Position Overview

This full-time Physical Therapist (PT), Home Health, position offers an opportunity to provide comprehensive in-home rehabilitation services to homebound seniors and recently hospitalized patients in the Greenwich, CT area. As part of a boutique home health agency, the Physical Therapist will serve individuals who require skilled therapy to increase strength, improve range of motion, and support overall well-being during recovery. The Physical Therapist plays a vital role in delivering outpatient rehabilitation at home under Medicare Part A, with a focus on quality care and positive outcomes for patients adjusting to life after surgery or hospitalization.

Responsibilities

• Deliver skilled, evidence-based physical therapy to homebound patients under Medicare Part A and B, with a primary focus on seniors and post-acute cases.
• Conduct patient assessments, establish individualized care plans, and implement therapeutic interventions to facilitate optimal recovery and independence.
• Document patient progress and therapy outcomes in accordance with Medicare Part A standards, maintaining thorough and timely records.
• Coordinate care with interdisciplinary team members, including physicians, nurses, occupational therapists, and social workers, to ensure seamless patient support.
• Educate patients and their families about recovery processes, home exercise programs, and strategies for maximizing mobility and safety at home.
• Travel within the service area, including Greenwich and Westport, CT, to complete required weekly visits (27 visit equivalents, including starts of care and regular sessions).
• Maintain flexibility in scheduling to meet patient needs while fulfilling full-time visit requirements.

Qualifications

• Doctorate in Physical Therapy (DPT) or equivalent professional degree preferred; candidates with substantial homecare experience are encouraged to apply.
• Minimum of 5 years' experience in physical therapy, with a preference for those with a background in home health care and post-acute rehabilitation.
• Thorough understanding of Medicare Part A and B guidelines and documentation standards.
• Ability to work independently, demonstrating clinical judgment, compassion, and strong organizational skills.
• Valid physical therapy license for the state of Connecticut.
• Valid driver's license and reliable transportation for travel between patient sites in Greenwich and Westport, CT.
