Physical Therapist (PT) - Summary
- Deliver individualized physical therapy services to patients in alignment with established care plans.
- Support patient rehabilitation, focusing on achieving optimal functional outcomes and overall well-being.

Duties & Responsibilities
- Provide high-quality physical therapy treatment and interventions as detailed in patient care plans.
- Assess, monitor, and document patient progress, adjusting therapy plans as needed to achieve established goals.
- Educate patients and caregivers about therapeutic exercises, treatment plans, and strategies to maximize recovery and prevent injury.
- Communicate effectively with patients, families, and interdisciplinary team members to ensure coordinated and comprehensive care.
- Maintain accurate and timely clinical documentation in compliance with regulatory standards and organizational policies.
- Adhere to all safety protocols and professional guidelines to protect the well-being of patients and staff.
- Utilize reliable transportation to travel to patient locations as required.

Qualifications & Requirements
- Current and valid Physical Therapist (PT) license as required by applicable state regulations.
- Certification in CPR from the American Heart Association or the American Red Cross.
- Minimum of one year of experience working in a healthcare facility, preferably providing direct patient care.
- Strong physical stamina and the ability to meet the physical demands of the position.
