Physical Therapist (PT) - Summary
- Deliver patient-centered physical therapy services in home settings
- Work collaboratively as part of an interdisciplinary team

Duties & Responsibilities
- Provide direct patient care, including comprehensive evaluations, assessments, and developing treatment goals
- Design and implement individualized treatment plans based on clinical assessments and patient/family input
- Monitor and document patient progress, adjusting care plans as needed to achieve optimal outcomes
- Consult and collaborate with physicians, nurses, occupational therapists, and other healthcare providers
- Educate patients and caregivers on therapeutic exercises, safety, and strategies for maximizing mobility and independence
- Supervise and direct support staff to ensure consistent delivery of care and achievement of patient goals
- Maintain accurate records, including evaluations, treatment notes, goals, and prescriptions
- Ensure all care is provided in accordance with established standards, regulatory requirements, and Elara Caring policies
- Serve as a resource and leader within the team and in the community to promote awareness of physical therapy

Qualifications & Requirements
- Graduate of an American Physical Therapy Association (APTA) accredited program
- Current, unrestricted Physical Therapist (PT) license valid for the state of employment
- Minimum of one (1) year of experience working in a healthcare facility
- Ability and willingness to travel within assigned coverage area to provide patient care
- Dependable vehicle, valid driver’s license, and current auto insurance
- Commitment to travel as needed to meet patient and organizational needs
